The following forms are available from the school office or may be downloaded here in PDF format. 

We must have the parent consent form, and the physician authorization form if it is a prescription medication, completed before any medication is administered to your child at St. Eugene School. The medication, both prescription and over-the-counter, must be in its original container, clearly displaying the child's first and last name, name of medication, instructions on dosage and when it is to be given.
